How do I get started?

In order to get started in the program, we will need to obtain a referral from your provider. Simply provide us with your provider’s contact information, and we will be happy to obtain the referral on your behalf. If you do not have a provider, one of the providers at UT Health Services will meet with you and complete your referral form. You can find the Referral Form in the link at the bottom of the page.

What is Diabetes Self Management Education (DSME)?

DSME is an educational program designed to help people with diabetes and their families gain the knowledge and skills needed to modify behavior and successfully manage the disease. The University of Texas Health Services (UTHS) diabetes program is recognized by the Association of Diabetes Care and Education Specialists (ADCES). This recognition assures that the program meets the highest standards outlined by AADE for providing education to patients with diabetes. Our registered dietitian and certified diabetes educator teaches the classes in an interactive group environment. The program covers the seven self-care behaviors that are important to focus on to live a healthy life:

  • Healthy Eating
  • Being Active
  • Monitoring
  • Taking Medication
  • Problem Solving
  • Healthy Coping
  • Reducing Risk
